This is one of the Trinket Boxes that can be found on CD 2.
There are a selection of different ones that you can make and everything you need is on the CD. I printed mine onto Crafter's Companion White card stock and used Collall Tacky Glue to make it up.
I used elements from CD 1 to decorate my box.

Oh, I'd love to have this cd, I love to travel, and I work within the travel trade, so this is like a must have. But the thing is it will cost a fortune if I buy anything abroad exceeding 30 pounds postage cost included. But I hope I can get this from a supplier in Norway in the future. I love what you've made!!
